CARD 1. What is Facial Contouring Surgery?

Facial contouring surgery is a cosmetic surgery in which facial bones are cut or moved to the desired position for aesthetic purposes. In cases of severe facial asymmetry, large face, or uneven facial line, patients often have trouble choosing to go between a plastic clinic and an oral and maxillofacial surgery clinic.

Ms. Y (25 years old), who was dissatisfied with her wide face, had undergone a square jaw reduction, genioplasty, and zygoma reduction at the E oral and maxillofacial surgery clinic two years ago.As swelling disappeared, the right temple became depressed, and the cheeks began to sink. Besides, facial asymmetry, where the zygoma is more prominent with the left lower jaw looking larger, aggravated. The E clinic insisted that this is due to temporary muscle contraction and swelling. They also promised a revision in the future if asymmetry persists, but now they are stating that it is unnecessary.

CARD 3. Facial contouring surgery: Plastic surgery vs. Oral and maxillofacial surgery

Through 3D computed tomography (CT) images and panoramic X-ray examinations, it is possible to determine the degree of asymmetry and how much facial bones can be reduced. If the jaw or zygoma is large, or only one side is exceptionally developed, an operation in the plastic surgery field is necessary. However, the asymmetry with problems in the central axis of the face or teeth malocclusion needs an oral and maxillofacial surgery consultation.

CARD 4. Onset point of congenital facial asymmetry

In infancy, before ages 4 to 5, facial asymmetry appears in the head, forehead, and eyes. In childhood, between ages 5 to 12, it appears on the center, such as the nose and cheekbones. Next, asymmetry develops on the mandible during adolescence over 12 years of age. After adolescence, the central axis of the face gradually bends, and the asymmetry becomes severe due to the asymmetry of the lower jaw. Many tend to be treated in adulthood.

CARD 5. Facial Asymmetry that Needs Double Jaw Surgery

For self-diagnosis, horizontally hold a pen deep in your mouth and see if the corners of your mouth are either parallel to each other or severely tilted to one side. If the dental midline is notably tilted or malocclusion is severe, then double jaw surgery may be considered.

If you feel asymmetry only when you laugh or talk, then the asymmetry is likely due to the soft tissues and muscle development. You can expect improvements by simply exercising facial muscles like opening your mouth wide and repeating, “A-E-I-O-U,” or practicing singing while smiling and looking in the mirror.

CARD 7. Facial Asymmetry after Facial Contouring Surgery

The central axis of the face is not bent or deformed due to facial contouring surgery. However, this may occur if surgery is performed without accurately checking the degree of facial asymmetry before surgery, or if the amount of bones reduced on the right is different from left. It can be fixed through re-operation. But, fat transplantation, liposuction, or lifting procedures can be considered, depending on the condition like an asymmetry caused by soft tissues.

CARD 8. Depressed temple and Sunken cheeks after facial contouring surgery

This is highly related to zygoma reduction. Temporal depression may appear when the fat tissue of the temple adjacent to the zygoma is damaged during surgery. Sunken cheeks may appear temporarily right after the surgery. However, if the symptoms of cheek depressions are severe or aggravating over time, then the causes may be ▲ overcorrection in zygoma reduction surgery, ▲ incorrect fixation of pin positions when reducing zygoma, ▲ zygoma retaining ligament or tissue damage.

In brief, filler treatment or fat grafting on the depressed temples and cheeks improves the symptoms. On the contrary, if the damage of zygomatic ligaments or tissues is the cause, then muscular dissection (the process of finding and separating layers between tissues) can be attempted through an incision inside the mouth. It is good to consider corrective surgery when you are not satisfied with a simple correction. Lifting procedures would be helpful if sunken cheeks occurred due to skin laxity.
