Card 1 The more bone is shaved, the prettier you become?

Facial contouring surgery can certainly make a small and slim V-line facial shape. Yet, many people worry about the awkwardness and saggy skin, which appears as they age, after having the surgery with the wrong idea that cutting a lot of bones means more beautiful. Dr. Park Sang-hoon at ID Hospital spoke about skin sagging and shape dissatisfaction after facial contouring surgery.

Card 2 A real case of skin sagging after facial contouring surgery

A 31-year-old fashion model (for online shopping), Ms. A underwent facial contour surgery at “G” plastic surgery in Gangnam eight years ago. She was satisfied after receiving a corticotomy—a thickness-reducing operation, resecting the total lower jawbone from under the ears to the front jaw. However, as time went by, the whole lower jaw began to sag, and it felt like she has a double chin. Also, the awkward jawline became noticeable enough for anyone she met for the first time to realize she had plastic surgery.

Card 3 The cause of saggy skin after facial contouring surgery

Bone surgery requires a detachment process that separates the bone from the periosteum surrounding the bone. The skin sagging, which inevitably occurs during this process, increases with the surgery area. Also, excessive resection can be another reason if the skin elasticity is low, or there are a lot of soft tissues depending on the skin characteristics of the individual.

Card 4 Saggy skin depending on the surgical area

Cheekbone reduction is an operation at the center and ligament-retaining area of the face, so if it is not firmly fixed, the skin is more likely to sag. Skin sagging can occur after square jaw surgery in case of low skin elasticity or long facial shape.

Card 5 Preventing skin sagging from facial contouring surgery: ①Decide surgical method based on your facial shape

Consider the overall balance of facial and head shape, and then find a surgical method suitable for you. Those with a short, square-shaped face can be satisfied with overcorrection for less saggy skin and a slimmer face. But people with longer faces have a higher chance of skin sagging, and their faces may look even longer, and that overcorrection must be avoided.

Card 6 Preventing skin sagging from facial contouring surgery: ②Surgical process

It is important to ensure that there is minimal detachment and no damage in facial retaining ligaments. Besides, the suturing process, which fixes the periosteum, should be done carefully.

If there are a lot of soft tissues on the skin, liposuction is recommended. For low skin elasticity, skin sagging can be prevented to some extent by performing thread lifting and facial surgery simultaneously.

Card 7 Preventing skin sagging after facial contouring surgery: ① Self-care in daily life

Self-care is very important during the three months when bones and periosteum reattach. Right after the surgery, a face band should be used for a certain period. The upper part of the body should be raised high to prevent skin sagging caused by edema and bleeding.

Card 8 Preventing sagging skin after facial contouring surgery: ② Laser or Lifting

High intensity focused ultrasonic (HIFU) laser treatments such as “Ulthera” and “Shurink” are expected to be beneficial if done once or twice. However, if skin sagging is severe, or if the laser treatment isn’t satisfying, it is recommended that you first try a relatively simple method before having other procedures: thread lifting with cogged threads, minimal incision face lifting, face lifting surgery.

Card 9 After facial contouring, does saggy skin get worse as you age?

After three months of reattachment of the bones and periosteum, the skin sagging from the surgery does not worsen, and there is no difference from natural aging. Since the volume of soft tissues remains while that of bones gets reduced, people do not feel skin sagging only in their 20s.

Card 10 When is corticotomy needed?

Lower jaw contouring surgery is an operation that reduces the thickness of the jawbone by resecting the cortical bone—the outermost part of the bone. It gives a slimming effect when viewed from the front after surgery, but excessive resection can cause uneven surfaces or nerve damage, so it is no longer frequently performed. Also, minimal corticotomy is advised even if it’s necessary.

Card 11 How to improve straight jawline from under the ears to the frontal jaw

It is a side effect dubbed “dog-chin.” If the jawline is uneven, an outline can be created by volumizing with fillers or autologous fat injection. For the thick and wide neckline, effects can be expected from simple methods such as "facial contouring injection" for removing fat under the chin, or "botulinum toxin treatment" for decreasing salivary glands. Implants may be used for more reliable effects, but there is a risk of inflammation from foreign substances, external shock, and implant movement—so it should be carefully determined.

Card12 Advice: Dr. Park Sang-hoon at ID Hospital, advisory committee member of K-Health

“Facial contouring surgery should be decided very carefully because there are many things to consider in advance, such as head and facial shape and skin elasticity. Also, it is important not to resect too much bone during the surgery, and it is best to prevent other adverse effects by trying to improve them with the easiest way possible in case of skin sagging or shape dissatisfaction.″
